The St. Mark’s Parish Suggestion Form will be used as a communications tool that will allow parishioners to voice ideas and suggestions to one of the Standing Commissions or to the Parish Pastoral Council. Forms will be located on the website and in the kiosks at the back of church. All forms received will be addressed in a timely manner.
Indicate which commission or parish council to whom the form is directed.
If unsure, check Parish Pastoral Council who will deliver it to the appropriate group.
Indicate if you wish to present information during a commission or council meeting.
Indicate name, address, phone number and email address. This will allow the recipient to
contact the individual to ask questions or seek further explanation and provide a response
after the idea is considered.
Complete information regarding the subject/issue, narrative, benefits, and costs.
Indicate willingness to assist with request. You may also

deliver it to the parish office. Forms submitted via the website will be routed directly to the Pastoral Council Vice Chairperson’s email. Forms submitted to the parish office will be faxed to the Pastoral Council Vice Chairperson.
